Wound Care

Diabetics must be wary of all wounds, regardless of depth or size. Diabetes, a chronic disease in which the body cannot properly use glucose the way it normally would, causes various complications that make wounds difficult to heal. Nerve damage or neuropathy will cause diabetics to have trouble feeling the pain of a blister or cut until the condition has significantly worsened or become infected. A diabetic’s weakened immune system can make even the most minor of wounds easily susceptible to infection. Diabetics are also more prone to developing narrow, clogged arteries, and are therefore more likely to develop wounds.

Wounds should be taken care of immediately after discovery, as even the smallest of wounds can become infected if enough bacteria build up within the wound.  To remove dirt, wounds should be first rinsed under running water only. Soap, hydrogen peroxide, or iodine can irritate the injury and should be avoided. To prevent infection, apply antibiotic ointment to the wound and cover it with a bandage. The bandage should be changed daily. The skin around the wound may be cleaned with soap.

To prevent further exacerbation, see a doctor—especially if you have diabetes. Minor skin conditions can become larger problems if not properly inspected. As the wound heals, make sure to avoid applying pressure to the affected area.

Arches of the Foot

The three arches of the feet support body weight and help absorb shock during movement. They are the medial and lateral longitudinal arches, which run from heel to toes, and the transverse arch, which spans across the midfoot. The medial arch is higher and more flexible, while the lateral arch is flatter and more stable. Understanding Mueller‑Weiss Syndrome

Mueller‑Weiss syndrome is a rare condition that affects the middle of the foot, but podiatrists are seeing it more frequently. It occurs when the navicular bone, located at the top of the arch, loses its blood supply and begins to collapse. This can lead to chronic foot pain, stiffness, and changes in the shape of the foot. Although the exact cause is not fully understood, it may be linked to abnormal foot structure, past injuries, or uneven pressure on the foot over time. Most patients are adults, often women in their forties to sixties, and the pain usually develops slowly. Because it can be mistaken for other foot issues, a proper diagnosis is important. Treatment may include custom orthotics, special shoes, targeted exercises, or surgery, in severe cases. If you have ongoing midfoot pain, a podiatric evaluation is suggested.

Rare Foot Conditions

The majority of foot conditions are common and can be treated by a podiatrist.  Standard diagnostic procedures are generally used to identify specific conditions and treatment can be rendered. A podiatrist also treats rare foot conditions which can be difficult to diagnose and may need extra attention and care. 

There are many rare foot conditions that can affect children. Some of these can include:

  • Freiberg’s disease
  • Kohler’s disease
  • Maffucci syndrome

Freiberg’s disease - This can be seen as a deterioration and flattening of a metatarsal bone that exists in the ball of the foot. It typically affects pre-teen and teenage girls, but can affect anyone at any age. Symptoms that can accompany this can be swelling, stiffness, and the patient may limp. 

Kohler’s disease - This often targets the bone in the arch of the foot and affects younger boys. It can lead to an interruption of the blood supply which ultimately can lead to bone deterioration. The patient may limp or experience tenderness, swelling, and redness.

Maffucci syndrome - This affects the long bones in a child’s foot leading to the development of abnormal bone lesions. They are benign growths and typically develop in early childhood and the bones may be susceptible to breaking.

Rare Foot Conditions

The metatarsal bone becomes deteriorated and flattened, and common symptoms include swelling and stiffness. A cast is often necessary to wear with this disease as it can help to reduce existing pain. Erythromelalgia is a rare foot condition, and its cause is unknown. Symptoms of this disease can include intense burning pain and the feet may appear red or feel warm. Relief may be found when the affected foot is immersed in ice water. It can also be beneficial to elevate the foot frequently. Peripheral Artery Disease and the Feet

Peripheral artery disease, or PAD, is a condition where the arteries that carry blood to the legs and feet become narrowed by a buildup of fatty deposits. This limits blood flow, especially when walking. A common symptom of peripheral artery disease is cramping or aching in the calves after a short distance, which usually stops with rest. This happens because the muscles are not getting enough oxygen-rich blood. People with PAD may also notice slower toenail growth, less hair on the lower legs, or feet that feel cool to the touch. In more serious cases, pain can develop in the feet even at rest, particularly at night, and sores may appear on the toes or feet due to poor blood supply. A podiatrist can check for weak blood flow in the feet and may use a simple ankle pressure test to help diagnose the problem. Peripheral artery disease (PAD) is when arteries are constricted due to plaque (fatty deposits) build-up. This results in less blood flow to the legs and other extremities. The main cause of PAD is atherosclerosis, in which plaque builds up in the arteries.

Symptoms

Symptoms of PAD include:

  • Claudication (leg pain from walking)
  • Numbness in legs
  • Decrease in growth of leg hair and toenails
  • Paleness of the skin
  • Erectile dysfunction
  • Sores and wounds on legs and feet that won’t heal
  • Coldness in one leg

Diagnosis

Peripheral Artery Disease

Peripheral artery disease (PAD), or peripheral arterial disease, is a circulatory problem in which there is a reduction of blood flow to the limbs due to narrowed arteries. When peripheral artery disease develops, the extremities do not receive enough blood flow; this may cause symptoms to develop such as claudication, or leg pain when walking. The legs are the most common site of peripheral artery disease.

Claudication, or leg pain when walking, is one of several symptoms that can develop due to peripheral artery disease. Other symptoms caused by the disease include painful cramping in the hips, thighs, or calves after certain activities; leg numbness or weakness; coldness in the lower leg or foot; sores on the lower extremities that do not heal; hair loss on the lower extremities; and a missing or weak pulse in the lower extremities. In more severe cases, pain may even occur when the body is at rest or when lying down.

Peripheral artery disease is typically caused by atherosclerosis, a condition in which fatty deposits build up in the arterial walls and reduce blood flow. Smoking, diabetes, obesity, high blood pressure, and high cholesterol are some of the risk factors for peripheral artery disease.
